I understand the organization's maintenance of its charitable activities and federal tax exemption depends on its undertaking primarily of activities that support its charitable purpose; and

I do not have any on-going relationships or interests that present an actual or potential conflict of interest (as defined in Article 4(a) & (b) of the Policy) other than as described below.
